Pain in the butt……..and leg literally

This is in response to people asking about where have I been past 4 weeks , since I have not been the most compliant returning calls, texts. Or emails. So this is the most logical + easiest way to respond. This is not a pity party-just hopefully a minor bump on the road. I am fine considering everything

October 1, 2012 note

After working a 12 hr. shift in an Urgent Care Center in Scranton, during a short period of rain, I slipped and fell in a parking lot .Initially, only my pride was bruised from my less than graceful fall;
However, by the time I got home from my 2 hr. drive, I started developing low back, (R) hip, (R) anterior. Tibia pain.. This is not good when you have had a laminectomy 30 years ago and a discectomy 15 yrs. ago.

My situation today is, after 1 week of strongly recommended Bed rest and avoidance of interaction with people (if possible) to reduce stress, I am still experiencing moderate amount of sciatic pain on siting after 1 minute. Sucks being a patient.

I have seen a chiropractor (massage +stretching), discussed case with Neurology (prescribed a muscle relaxer), saw Pain Specialist (trigger point injections), Rx mild. pain med) ,Still on tapered dose of Prednisone, and finally talked to Neurosurgeon(was part of group that did my discectomy 15 years ago)

Now I have to get an MRI to decide exactly the Diagnosis, and, if it is a Herniated Disc that is putting pressure on the nerve, than need to take care of it.

If MRI is negative, then great ( I wish) and i will deal with pain.

Dilemma is No health insurance, No Disability Insurance (working as Locum Tenens independent contractor, these benefits are not offered ) Doing Ok and soon, this minor problem shall pass also, and I will again be able to sit up again pain-free.

Monday 10/15 Progress note –
Had a MRI on Thursday, visit with Neurosurgeon on Friday.
Good news-no surgery at this time
Bad news-Per neurosurgeon-I have a very shitty back.
MRI reveals multiple severe foramina stenosis of Lumbar spine causing inflammation of nerves=sciatica multiple areas.
(stenosis=narrowing of spine)
So why couldn’t it be narrowing of my abdomen instead of spine?So unable to sit or stand for more than a couple of minutes , but able to finally sleep thru night last night first time.
Now , been told it will take a couple of weeks and hopefully pain will decrease.
Will need to look for new work soon-no income or disability will make this challenging.
So I will be hanging out+ if any one wants to help this old, pathetic patient recover, I ain’t proud (my pride has been taken away years ago)

Summer in the City 2012 -update

Well, I continue to meet rough waters in my journey trying to return to a comfort medical setting that allows quality , integrity and fun interaction with staff+patients be a major priority in a work environment , while surviving financially .
So far I have struck out. Just like the Phillies.

Right now I am working for another Urgent Care, this in Philadelphia, and I am not planning to move their if possible-too far away.
I am still crazy enough to want to start back in the Central Bucks area, especially when I am still getting calls from patients wanting me to be their doctor.Either YOU GUYS are as crazy as me, or the quality of care is lacking or you just miss being around a round, jovial Wandering Jewish doctor.

The dilemma is that I am still flat broke due to the state beating me up still due to a FALSE ACCUSATION , and they are just a tad more powerful and have just a tad more money (hell , a tree has more money than me), so I am not sure what to do.

From my multiple jobs(3 more since the Russian experiment) I am disappointed in the quality of care some providers are giving.The most frustrating thing is that many of the medical decisions are being made by non medical people, just for the sake of cutting costs.

So I am still looking for options , and I have tried to get some of the Urgent Care companies to start-up a facility in the Warrington region, but , so far , no one has committed. That or I need to find a blind , wealthy woman or win a lottery. Yeah, right!!!

This has been an update and hopefully we will all survive the heat of the summer and the coldness of the corporate run world.

Another Russian Connection

As most know , I haven’t had my own car for past year due to circumstances, credit reports and lack of certain documents from certain work places from . So I have helped the car rental companies economy much more than I should have , but I did not have much choice.

Well, my tour thru the car dealerships was somewhat eye- opening, frustrating and costly. Just as I am slowly getting my credit scores better, multiple dealerships either misled, offered me not the best deals or rejected me while continuously applying to multiple banks-ie., bringing my scores down .Another painful lesson in the school of life

Well, I got lucky yesterday and purchased a quality car from Connicelli’s dealerships. And the gentleman who is specially trained for “bad credit” was another Russian named Dmitri, and he was great. Most of you know that I don’t rush to complement(old staff members, no comment please) , but this guy didn’t B.S. me . After evaluating really what was my situation, he got a great rate for me that was at least 1/3 to 1/2 of rates in other places.

And to top it off, he multitasks well, can hyper focus and does get distracted at times, thrives in chaos—-hhmmmmmmmmmmmmm, I wonder what he has?

So , at least for a day, life got better. I am still in the state monitoring program and still not thrilled , but I only have one more year left .I was sent back to legal, due to some missed screens, but if someone asked me 5 years ago that I would possibly miss an occasional random test , would I be surprised , well the answer is I would not be surprised due to my ADD and life’s challenges. Not much I can do but keep doing the best I can with the cards dealt me. IT IS WHAT IT IS…

I am still in the Northeast , but there only 4 days/week , and not sure where my next destination will be, but why should life be simple..

Here we Go Again-I am working the United Nations tour-Initially Koreans, Chinese(Taiwan -Dr Cheng) then Iranian, and now finally Russians –

I have taken another detour in my attempt to return to my professional roots in the beautiful , downtown Central Bucks (obviously my journey has done some injury to my brain). Plus , I continue my medical journey of treating patients and/or employers that are different nationalities

I have left the Urgent Care Centers that were owned by the Iranian physician a couple of weeks ago due to some basic differences. One, is that he still owes me a paycheck; and, number 2 , his treatment of people , plus his ethics were much different from mine.

I started working in a Physical Medicine Rehab center in the Northeast 3 weeks ago. owned by a Mad Russian (actually not a bad guy , and as almost crazy as me ) .

Needless to say, Life is not boring (Thank God)

Most of the patients and staff talk Russian and, I , of course, have a hard time just with English , let alone Russian. So this has been unique, but everybody is pretty cool and nice to me( they don’t know me that well yet) And, really, how many people have ever listened to me ? Staffs don’t answer that. Right now I am working Part time with possibily doing full time in near future.

The nice thing here is that there 2-3 massage therapists,a chiropractor and an acupuncturist, all alternative treatments dealing with the physical and mental aspects of life.

I just became Medicare participating, but other insurances will take time. Right now , A cost of an office visit should be affordable (hell, I am not doing brain surgery-hmmmm) while the group does participate in most insurances for the services offered here..
Right now , the office hours are 10 AM to 5 PM and probably a night will be added in the future. This format is going to be low-keyed ((not crazy like the old days) , with special emphasis on mind-body connection. House calls can be done if time is available, and Medicare will cover that.

I am still restricted in prescribing stimulants for Adult ADD by the state monitoring agency at this time. Dr. Boyd , to his credit , acknowledged in a letters and in actions , that his original diagnosis of addiction /substance abuser , was totally wrong.. Unfortunately, the powers to be, today , haven’t changed their opinion for this incorrect accusation . Yes, I am still paying the price for being a strong advocate of Adult ADD as a family doctor.

So who knows how this new opportunity turns out , but after being in Chicago, State College , Scranton and Reading, at least I am getting closer to my stomping grounds.

One of my friends asked,”Are you ready for the Russians? The fact is that wherever I worked there good are and bad people who I have had to deal with , and I am sure it will be the same. So, yes , I am ready for the Russians. The bigger question is “Are the Russians ready for me?”
